Understanding Maxillary Sinusitis of Endodontic Origin (MSEO)

Many patients suffer from chronic or recurring sinus issues that don’t fully resolve with standard ENT treatments. What’s often overlooked is that some of these cases start in the teeth.

The roots of the upper premolars and molars lie very close to (or even inside) the maxillary sinus floor. When a tooth has necrotic pulp or an untreated canal, infection can spread directly into the sinus. This is known as La sinusitis maxilar de origen endodóntico (MSEO).

Common Signs That Suggest a Dental Cause:

  • Chronic or recurrent unilateral (one-sided) sinus symptoms
  • Persistent facial pressure or pain in the cheek/upper teeth area
  • Foul odor or taste (halitosis)
  • Symptoms that improve temporarily with antibiotics but keep returning
  • History of dental work on upper back teeth

Studies show that odontogenic causes account for a significant percentage of maxillary sinusitis cases, rising even higher with unilateral presentations.

How We Accurately Determine If There Is a Dental Cause

Advanced Diagnosis with Cone Beam CT (CBCT)

Unlike traditional 2D X-rays, our in-office cone beam imaging provides detailed 3D views of the teeth, roots, and sinus floor. This allows us to:

  • Identify periapical infections, untreated canals, or root fractures that standard imaging often misses
  • Visualize mucosal thickening, fluid, or other sinus changes directly related to a tooth
  • Differentiate dental sources from purely sinonasal issues

Clinical Evaluation Includes:

  • Detailed history and symptom review
  • Microscopic intraoral examination
  • Assessment of previous root canal treatments

Early and accurate diagnosis prevents unnecessary sinus procedures and improves overall outcomes.

Our Advanced Treatment Approach

When a dental source is identified, we provide precise, conservative endodontic care focused on saving the natural tooth and clearing the infection.

Our Technology Includes:

  • Surgical Operating Microscopesfor unmatched visualization of complex root canal anatomy
  • GentleWave Procedurean advanced system that uses acoustic energy and optimized fluids to remove bacteria and biofilm more effectively than traditional methods

Treatment Goals:

  • Eliminate the source of bacterial infection
  • Preserve your natural tooth
  • Promote healing of the surrounding bone and sinus tissues

En la mayoría de los casos, thorough endodontic treatment alone leads to resolution of the sinusitis. When further sinus care is needed, we collaborate closely with your ENT.
